LORHN staff are trained Leaders for the Chronic Disease Self-Management Program (CDSMP), an evidence-based program originally developed at Stanford University. Workshops are offered to rural residents for free, in six-week series with up to 15 participants meeting weekly for 2.5 hours. This workshop focuses specifically on diabetes self-management for the full six weeks. Though much of the content and skills taught are similar to the Chronic Disease Self-Management workshop, there are also variations specific for individuals living with diabetes, for example:
- monitoring
- avoiding complications
- stress management
Spouses, partners, caregivers, and adult children are welcome to join participants in the series.
